Waiver & concussion acknowledgement

I am the parent or legal guardian of the player named above. I understand ice hockey and on-ice training involve inherent risks of injury, and I accept those risks on the player's behalf. The player will wear CSA-certified helmet with full cage, neck guard, gloves and skates for every on-ice session. I release Vaughan Ice Works and the platform from claims arising from ordinary risks of participation, except where caused by negligence.

Rowan's Law: I confirm that I and the player have reviewed the Government of Ontario concussion awareness resources within the past 12 months, and that the player will be removed from activity if a concussion is suspected.

I accept the waiver and the Rowan's Law acknowledgement.
